New Patient Info

Chiropractic Nutrition Center offers our patient form(s) online so you can complete it in the convenience of your home or office.  You must obtain a new patient pin number from our front office prior to filling out these forms. Call 919-461-0046 to obtain your pin number, then click the link below to begin filling out the forms.

Before the first visit

Before you come for your first, you will be able to fill out the paperwork online. Filling out any available paperwork before your visit is a great convenience and can save you time.

You can call our office to get a patient pin number and access the paperwork on this website, or we will also email you a link to the paperwork before your first visit.

The First Visit

Once all paperwork is filled out, you will meet with one of our doctors. During this time, the doctor will conduct a brief history and examination. This will help us further understand your condition.

If x-rays are necessary, they will also be taken during this visit. You can expect this visit to last anywhere from 30 minutes to an hour. A second appointment will then be scheduled at your earliest convenience.

The Second Visit

The doctor will sit down with the information he gathered at the examination and create a personalized treatment plan based on your specific condition and needs.

You will be fully briefed on treatment plan options. Once you feel that you have a good understanding of your condition and we have answered all of your questions, you will be treated.

Treatments

The most common form of treatment in our office is the “diversified chiropractic adjustment,” which involves specialized movements of the spine to restore motion and decrease pain.

Other treatment options include a variety of modalities such as muscle work/stretching, traction, pro-adjuster, and electric stim. You can expect a combination of treatments to be used for maximum results.

Re-Exams

These are done every so often to evaluate how you are responding to treatment. These are important in all medical fields in holding doctors responsible for monitoring patient outcomes.

If by chance you are not responding favorably, we will make sure your treatment is modified, or we will refer you to another professional who will be able to treat you further.
